  Peau endommagée par le soleil 
Tout le monde bénéficie de la chaleur et de la lumière du soleil. Elles sont essentielles à la production de vitamine D par le corps et peuvent influencer votre humeur. Cependant, une exposition excessive au soleil peut causer des dommages à la peau. Les rayons ultraviolets (UV) peuvent provoquer des brûlures et des changements à long terme. Les types de dommages les plus courants à la peau sont :
  • Peau sèche : La peau exposée au soleil peut progressivement perdre son hydratation et ses huiles, ce qui la fait paraître sèche, squameuse et ridée prématurément, même chez les jeunes.
  • Coup de soleil : Le coup de soleil est le nom courant des dommages qui surviennent immédiatement après une exposition excessive de la peau aux rayons UV. Un coup de soleil léger provoque seulement une rougeur de la peau, mais les cas plus graves peuvent présenter de petites cloques remplies de liquide ou des ampoules plus grandes.
  • Kératose actinique : Ce sont de petites plaques de peau endommagée par le soleil qui varient en couleur - rose, rouge, jaune ou brunâtre. Elles sont un signe d'alerte d'un risque accru de cancer de la peau. Environ 10 à 15 % des kératoses actiniques évoluent finalement en cancers cutanés épidermoïdes.
  • Photo-vieillissement (vieillissement prématuré) : La peau développe des rides et des ridules dues à des modifications du collagène dans la couche profonde de la peau appelée derme.
  • Purpura actinique : Cela résulte des dommages causés par les rayons UV au collagène structurel qui soutient les parois des vaisseaux sanguins de la peau. Les vaisseaux deviennent plus fragiles et se rompent facilement au moindre impact.

Les épisodes répétés de coups de soleil et l'exposition au soleil sans protection augmentent le risque de mélanome malin et d'autres formes de cancer de la peau. Les personnes à peau claire sont les plus exposées aux dommages causés par le soleil. Leur peau contient moins de pigment appelé mélanine, qui aide à protéger la peau des rayons UV. Selon un article publié par les National Institutes of Health, seulement cinq coups de soleil au cours de la vie suffisent à augmenter significativement le risque de dommages solaires et de cancer de la peau.

 Toute partie exposée de votre corps peut brûler, y compris les lobes des oreilles, le cuir chevelu et les lèvres. Même les zones couvertes peuvent brûler si les vêtements sont en tissu à tissage lâche laissant passer les rayons UV.

Signes et symptômes

Les signes et symptômes du coup de soleil apparaissent généralement quelques heures après l'exposition au soleil. Il peut falloir un jour ou deux pour que l'étendue complète des dommages soit visible.

Les signes et symptômes comprennent :

  • Rougeur
  • Sensation de chaleur ou de brûlure au toucher
  • Douleur, sensibilité ou démangeaisons
  • Gonflement
  • Petites cloques remplies de liquide qui peuvent se rompre
  • Maux de tête, fièvre, frissons et fatigue en cas de coup de soleil sévère

Causes et facteurs de risque

Les coups de soleil sont causés par une exposition excessive aux rayons UV. La lumière UV n'est pas visible à l'œil humain mais peut être très nocive pour la peau. Les rayons ultraviolets A (UVA) et ultraviolets B (UVB) sont les deux types de rayonnement les plus responsables des coups de soleil. Les lampes solaires et les cabines de bronzage produisent également des rayons UV et peuvent provoquer des brûlures de la peau.

Parce que les rayons ultraviolets sont la cause des coups de soleil, vous pouvez en avoir même par temps frais ou nuageux. Jusqu'à 80 % des rayons UV traversent les nuages. La neige, la glace, le sable, l'eau et d'autres surfaces peuvent réfléchir les rayons UV, brûlant la peau aussi gravement que la lumière directe du soleil.

Les facteurs de risque incluent :

  • Peau claire
  • Travail en extérieur
  • Activités de plein air avec consommation d'alcool
  • Antécédents de coups de soleil, surtout entre 15 et 20 ans
  • Exposition régulière de la peau aux rayons UV provenant de sources artificielles telles que les cabines de bronzage
  • Médicaments qui provoquent une photosensibilité de la peau

Les complications immédiates des coups de soleil incluent la rupture des cloques qui peuvent s'infecter. L'exposition au soleil et les brûlures répétées provoquent un vieillissement prématuré de la peau. Le photo-vieillissement affaiblit le tissu conjonctif en diminuant sa résistance et son élasticité. Le soleil rend la peau rugueuse et sèche. Une exposition prolongée au soleil forme également des rides profondes, des ridules et des veines rouges sur les joues, le nez et les oreilles. Elle provoque des taches de rousseur et des zones de taches foncées ou décolorées sur le visage, les bras, la poitrine et le haut du dos, appelées lentigos solaires.

Une exposition excessive au soleil, même sans coup de soleil, augmente le risque de cancer de la peau. Les coups de soleil pendant l'enfance et l'adolescence augmentent le risque de mélanome. Le cancer de la peau se développe principalement sur les zones du corps les plus exposées au soleil, notamment le cuir chevelu, le visage, les lèvres, les oreilles, le cou, la poitrine, les bras, les mains et les jambes.

Certains types de cancer de la peau apparaissent sous forme d'une petite excroissance ou d'une plaie qui saigne facilement et ne guérit pas. Un cancer de la peau peut aussi se présenter sous la forme d'un grain de beauté qui change. Consultez votre médecin si vous remarquez une nouvelle excroissance cutanée ou une excroissance qui change.

Traitement

 
  • Le traitement des coups de soleil ne guérit pas la peau ni ne prévient les dommages, mais il peut réduire la douleur et le gonflement. Ces conseils peuvent diminuer votre douleur et votre inconfort :
  • Prenez un analgésique, comme l'ibuprofène ou le naproxène, pour aider à contrôler la douleur, jusqu'à ce que les rougeurs diminuent.
  • Refroidissez la peau avec une compresse humide, comme une serviette essorée dans de l'eau froide. N'appliquez pas de glace directement sur la peau – cela peut causer ou aggraver les lésions cutanées.
  • Appliquez une crème hydratante, une lotion ou un gel d'aloe vera, ou une crème à la cortisone. Cela peut diminuer la douleur et le gonflement, et accélérer la guérison. Une crème à l'hydrocortisone à faible dose (0,5 – 1 %) peut réduire le gonflement et accélérer la guérison. Si les cloques se rompent, lavez avec un savon doux et de l'eau, appliquez une crème ou une pommade antibiotique et couvrez avec un pansement humide.
  • Choisissez une crème hydratante propre et simple pour de meilleurs résultats.
  • Si des cloques se forment, ne les percez pas. Cela ralentit la guérison et augmente le risque d'infection.
  • Buvez beaucoup de liquides, surtout de l'eau. L'exposition au soleil, les coups de soleil et la chaleur augmentent la perte de liquide par la peau.
  • Traitez la peau qui pèle avec douceur. Continuez à utiliser un hydratant.
  • Protégez la peau brûlée d’une exposition supplémentaire au soleil.
  • Évitez d’appliquer des produits « caïne », comme la benzocaïne, utilisés localement pour réduire la douleur en engourdissant la peau. Ils ont été liés à une affection appelée méthémoglobinémie. Cela diminue la quantité d’oxygène transportée par le sang et ne doit pas être utilisé chez les enfants. Les adultes ne doivent pas dépasser la dose recommandée, ni la fréquence recommandée.

Prévention 

Utilisez ces méthodes pour prévenir les coups de soleil :

  • Évitez l’exposition au soleil entre 10 h et 16 h, lorsque les rayons du soleil sont les plus forts. Planifiez les activités en plein air à d’autres moments. Limitez la durée de votre exposition au soleil. Cherchez de l’ombre quand c’est possible.
  • Couvrez-vous. Portez des vêtements tissés serrés. Les vêtements foncés offrent plus de protection. Portez un chapeau à large bord.
  • Nourrissez votre peau quotidiennement avec un hydratant botanique 100 % pur.
  • Utilisez fréquemment et généreusement une crème solaire naturelle – choisissez des crèmes solaires qui ne contiennent pas de parabènes ni de produits chimiques à base de pétrole (pour plus d’informations, lisez notre rapport gratuit, « Les poisons que vous mettez sur votre visage tous les jours »). Quelle que soit la couleur ou le type de peau naturel, utilisez une crème solaire à large spectre, qui bloque à la fois les rayons UVA et UVB. L’Académie américaine de dermatologie recommande d’utiliser un produit avec un facteur de protection solaire (FPS) minimum de 30. Appliquez-le généreusement, toutes les 2 heures. Les Centers for Disease Control and Prevention ne recommandent pas les produits contenant également des insectifuges.   

Vos questions répondues

  1. J’ai déjà un bronzage. Cela me protège des coups de soleil, n’est-ce pas ? Les recherches montrent que quelques séances de bronzage en intérieur avant une exposition directe au soleil ne protègent pas une personne contre les coups de soleil répétés. De plus, les conséquences à long terme du bronzage et des coups de soleil répétés sont plus graves que tout bénéfice d’un bronzage de base.
  2. La crème solaire est-elle sûre pour mes enfants ? Avant l’âge de 6 mois, il ne faut pas appliquer de crème solaire. L’Académie américaine de pédiatrie recommande d’utiliser d’autres protections solaires, comme couvrir avec des vêtements et utiliser de l’ombre.
  3. Mes yeux ont une sensation de « grains de sable » quand je prends un coup de soleil. Mes yeux peuvent-ils aussi brûler ? Oui. Portez des lunettes de soleil avec des verres offrant la protection la plus élevée. Il est préférable de porter des lunettes de soleil qui épousent bien le visage, de style enveloppant.
  4. Dois-je consulter un médecin si mes yeux me font mal ? Si les analgésiques ne soulagent pas et que l’inconfort ne s’améliore pas, vous devriez consulter votre médecin.
  5. Je prenais toujours des coups de soleil quand j’étais adolescent. N’est-il pas trop tard pour faire quelque chose maintenant ? Vous devriez quand même prendre des mesures appropriées pour vous protéger du soleil afin d’éviter d’autres dommages.
  6. Le bronzage n’est-il pas bon pour moi ? Il n’y a aucun bénéfice pour la santé à bronzer. Moins de lumière solaire qu’un bronzage est suffisant pour que le corps synthétise la vitamine D.
  7. Est-il sûr de bronzer avant 10 h et après 16 h ? L’exposition au soleil peut endommager la peau et provoquer des brûlures même pendant les heures où le soleil est moins intense.
  8. J’ai des stries rouges qui rayonnent à partir d’ampoules rompues. Qu’est-ce que c’est ? Une infection a peut-être pénétré les ampoules rompues et s’est propagée. Consultez votre médecin.
  9. Quand dois-je consulter un médecin pour un coup de soleil ? Vous devez consulter un médecin si la peau est cloquée et couvre une grande partie du corps, si vous avez de la fièvre, un mal de tête, de la confusion, des nausées ou des frissons, ou si cela ne s’améliore pas après deux jours.
  10. Comment savoir si c’est infecté ? Si la douleur, le gonflement et la sensibilité augmentent, ou s’il y a du pus qui sort d’une ampoule rompue, ou des stries rouges partant de la plaie, cela peut être une infection. Consultez votre médecin.
