La lentigo solaire est le terme médical pour les taches de vieillesse, parfois appelées taches de foie. Elles apparaissent le plus souvent sur la peau ayant subi la plus grande exposition au soleil : visage, cou, mains, bras, épaules et haut du dos. Les taches de vieillesse sont généralement plates et de couleur brune, noire ou grise. Elles sont courantes après 40 ans, mais peuvent survenir chez des personnes plus jeunes. Certaines peuvent ressembler à une affection médicale plus grave. 

Les taches de vieillesse sont inoffensives, mais elles peuvent susciter des inquiétudes concernant un cancer. Les lésions qui apparaissent spontanément, changent de forme ou croissent rapidement doivent être examinées pour détecter des bords irréguliers et des pigments variés au sein de la même lésion.

Comment éclaircir les taches de vieillesse

Pour des raisons esthétiques, les taches de vieillesse peuvent être éclaircies avec des agents dépigmentants ou retirées chirurgicalement. La meilleure gestion des taches de vieillesse est de les prévenir en utilisant un écran solaire et en évitant autant que possible l'exposition directe au soleil. Choisissez des écrans solaires naturels qui ne contiennent pas de parabènes ni de pétrochimiques.  

Causes

Les taches de vieillesse sont principalement causées par des années d'exposition aux rayons ultraviolets (UV) du soleil. Les lampes et cabines de bronzage commerciales endommagent également la peau. La mélanine est le pigment de la couche supérieure de la peau (épiderme), qui donne à la peau sa couleur normale. Les rayons UV accélèrent la production de mélanine, provoquant un bronzage qui protège les couches profondes de la peau des rayons UV.

Les taches de vieillesse se forment lorsque la mélanine « s'agglutine » ou est, dans certaines zones, en concentration particulièrement élevée. Le simple vieillissement peut également accélérer la production de mélanine, même sans exposition au soleil. Il peut y avoir un effet génétique, certaines personnes étant plus susceptibles que d'autres de former des taches de vieillesse. Les personnes à la peau claire sont plus sujettes à former des taches de vieillesse que celles à la peau naturellement plus foncée. Le risque est également accru en cas d'exposition solaire exceptionnellement intense et fréquente. (1,2)

Les lentigines solaires sont : 

  • maculaires - une zone plate et délimitée d'augmentation de la pigmentation de la peau qui n'est ni surélevée ni en creux.
  • généralement brunes, noires ou grises.
  • présentes sur la peau ayant subi le plus d'exposition au soleil au fil des ans, généralement sur le dos des mains, le dessus des pieds, le visage, les épaules et le haut du dos.
  • de taille variable, de 1 millimètre à 3 centimètres de diamètre.
  • susceptibles de se regrouper, ce qui les rend plus visibles.
  • parfois similaires en apparence à des lésions malignes ou non malignes.
  • plus susceptibles de se produire chez les personnes blanches ou asiatiques, en particulier chez celles ayant une tendance à avoir des taches de rousseur.

Le mélasma et les éphélides sont deux types de lésions maculaires hyperpigmentées non malignes qui peuvent être confondues avec des taches de vieillesse. Le mélasma est une hyperpigmentation progressive, non squameuse, de la peau exposée au soleil, en particulier sur le visage et les avant-bras.

La formation du mélasma est souvent associée à la grossesse, aux contraceptifs oraux et à certains anticonvulsivants. Il peut apparaître spontanément sans cause identifiable ni association à des facteurs connus. Il survient neuf fois plus fréquemment chez les femmes que chez les hommes, et plus souvent chez les types de peau courants en Asie, au Moyen-Orient et en Amérique du Sud. Il n'a aucune conséquence sur la santé, mais peut être gênant d'un point de vue esthétique. 

Le visage est le site le plus fréquent du mélasma, généralement au centre du visage ; il se manifeste moins souvent autour de la bouche et de la mâchoire. Les taches sont généralement bilatérales. Il existe trois types de mélasma : épidermique, dermique et mixte. Le mélasma épidermique est généralement brun clair et se révèle sous lampe de Wood. Le mélasma dermique est grisâtre et ne se révèle pas sous lampe de Wood. Les lésions mixtes sont brun foncé avec une réponse variable à la lampe de Wood. Les éphélides sont le nom médical des taches de rousseur, généralement de 1 à 2 millimètres, bien définies, sur le visage, le cou, la poitrine et les bras. (2) 

Les lentigines solaires peuvent ressembler à des grains de beauté (naevi), qui sont surélevés ou plats, et ne se limitent pas aux parties du corps exposées au soleil. Les kératoses séborrhéiques, qui sont non malignes, sont petites, de couleur fauve, brune ou noire, avec un aspect « collé ». Leur taille normale varie largement, de 1 millimètre à plus de 2,5 centimètres. Le lentigo malin est un type de cancer de la peau, un mélanome, qui peut se développer dans des zones d'exposition solaire prolongée. Il commence par une coloration fauve à brune puis noire, qui s'assombrit et s'agrandit. Ils ont des bords irréguliers et sont multipigmentés au sein de la même lésion. Les kératoses actiniques pigmentées sont prémalignes. (2,4) 

Les nouvelles lésions cutanées doivent toujours être évaluées par un dermatologue certifié. Les caractéristiques suivantes sont préoccupantes :

  • pigmentation foncée
  • croissance rapide
  • bords irréguliers
  • plusieurs couleurs au sein de la même lésion
  • sensibilité ou saignement
  • peau rouge et qui démange
  • douleur
  • mauvaise cicatrisation

Toute lésion atypique ou suspecte de mélanome doit être biopsiée, à travers toutes les couches de la peau. Les dermatoses séborrhéiques et les kératoses actiniques pigmentées ne peuvent pas toujours être distinguées par simple observation. En cas d'incertitude diagnostique, une biopsie doit être réalisée. Certaines maladies systémiques se manifestent d'abord par de multiples lentigines : syndrome de Peutz-Jeghers, syndrome LEOPARD et syndrome de Lamb.    

Prévention

Soin du corps  
  • Il est conseillé d'éviter l'exposition directe au soleil entre 10 h et 16 h, lorsque les rayons du soleil sont les plus intenses. Les activités en plein air devraient être programmées avant 10 h ou après 16 h.
  • Utiliser un écran solaire non seulement sur le visage, mais aussi sur des zones souvent oubliées comme le dos des mains, peut aider à prévenir les taches de vieillesse.
  • Un écran solaire, qui protège à la fois des rayons UVA et UVB, avec un facteur de protection solaire d’au moins 15, doit être appliqué 15 à 30 minutes avant l’exposition au soleil, puis réappliqué toutes les deux heures. Il est recommandé d’utiliser un écran solaire à large spectre avec un facteur de protection d’au moins 30.
  • Il doit être réappliqué plus souvent lors de la baignade ou de la transpiration. Il ne doit pas contenir d’ingrédients potentiellement nocifs tels que les parabènes ou les pétrochimiques.   • Couvrez-vous. Portez un chapeau à large bord qui offre plus de protection qu’une casquette de baseball ou une visière. Portez des vêtements tissés serrés, couvrant les bras et les jambes. Les vêtements conçus pour offrir une protection solaire sont utiles. Un facteur de protection ultraviolet (UPF) de 40-50 dans les vêtements est la meilleure protection. Le tissu peut perdre cette protection en s’étirant, en étant mouillé ou après plusieurs lavages. (2)

  Nourrissez vos cellules cutanées quotidiennement avec les nutriments dont elles ont besoin pour rester hydratées et éclatantes. Bien que les taches de vieillesse ne présentent aucun danger pour la santé, elles peuvent poser un problème esthétique. Des traitements sont disponibles pour les éclaircir ou les éliminer. La pigmentation se trouve dans la partie la plus profonde de l’épiderme, la couche supérieure de la peau. Tout traitement destiné à éclaircir les taches doit pénétrer jusqu’à la partie la plus profonde de l’épiderme. Ces traitements sont considérés comme cosmétiques, donc généralement non pris en charge par l’assurance. Il convient de choisir un médecin formé et expérimenté dans les différents traitements envisagés.  

  • Médicaments. Les crèmes éclaircissantes sur ordonnance (hydroquinone) utilisées seules ou avec des rétinoïdes (trétinoïne) et un stéroïde doux peuvent éclaircir progressivement les taches de vieillesse sur plusieurs mois. Il est fortement conseillé d’utiliser un écran solaire naturel avec un SPF de 30 (sans parabènes) lors d’un traitement médicamenteux. Cela peut provoquer des rougeurs, des brûlures, de la sécheresse ou des démangeaisons.
  • Thérapie au laser. La thérapie au laser détruit les mélanocytes, les cellules productrices de pigment, sans endommager la surface de la peau. Plusieurs séances sont généralement nécessaires. Après le traitement, les taches de vieillesse s'estompent progressivement, sur plusieurs semaines ou mois. Les effets secondaires sont rares, mais une légère décoloration de la peau peut survenir.
  • Cryothérapie (congélation). L'azote liquide ou un autre agent congelant est utilisé pour détruire le pigment excessif. Elle est généralement utilisée sur une seule tache de vieillesse ou un petit groupe de taches. Cela peut irriter la peau et il existe un léger risque de cicatrices permanentes ou de décoloration.
  • Dermabrasion. Ce traitement consiste à poncer la surface de la peau à l'aide d'une brosse tournante rapidement ; la nouvelle peau remplace l'ancienne. Il peut y avoir des rougeurs temporaires et la formation de croûtes.
  • Peeling chimique. Cette méthode de traitement consiste à appliquer un acide sur la peau, brûlant la couche externe. Lorsque la peau pèle, une nouvelle peau se forme. Plusieurs traitements sont généralement nécessaires pour voir des résultats. Une protection solaire est fortement recommandée après un peeling chimique. Il y a une irritation temporaire et un léger risque de décoloration.
  • Acide naturel. L'utilisation quotidienne d'un acide naturel efficace tel que l'acide lactique (issu du lait) peut aider à traiter l'hyperpigmentation. Il peut être utilisé quotidiennement dans un soin à domicile à faible concentration, ou en concentrations plus élevées dans un peeling chimique mensuel. Cependant, les acides doivent être utilisés uniquement sous la supervision d'un professionnel de la peau agréé, comme un esthéticien ou un dermatologue. Les effets secondaires incluent rougeurs, légère sensation de brûlure/inconfort, sécheresse et sensibilité accrue au soleil.

Mode de vie et remèdes maison

Les crèmes et lotions éclaircissantes sont disponibles en vente libre dans les grands magasins et les pharmacies, ainsi que sur internet. Elles peuvent améliorer l'apparence des taches de vieillesse au fil des mois. Les résultats dépendent en partie de la pigmentation des taches et de la fréquence d'application de la crème ou de la lotion. Le produit utilisé peut contenir de l'hydroquinone, du désoxyarbutine, de l'acide glycolique ou de l'acide kojique, qui peuvent provoquer une irritation cutanée. (4,2)

Il a été interdit dans les produits éclaircissants fabriqués en Europe et en Afrique, mais est encore utilisé dans un produit sur quatre fabriqué en Asie et vendu aux États-Unis. L'Organisation mondiale de la santé (OMS) avertit les consommateurs d'éviter le mercure car il cause des lésions rénales et réduit la capacité de la peau à se protéger contre les infections bactériennes et fongiques.

L'ingestion de mercure ou son absorption par la peau peut également provoquer de l'anxiété, de la dépression et même une psychose. Cela peut aussi causer une neuropathie périphérique - des lésions nerveuses, généralement aux mains et aux pieds. Cela modifie la sensation et peut provoquer des douleurs piquantes et brûlantes, réduisant considérablement la qualité de vie. Cela répond mal au traitement. (3)

Vos questions sur les taches de vieillesse répondues

 

Existe-t-il une pilule que je peux prendre pour faire disparaître les taches ? Il n'existe aucun médicament oral pour prévenir ou estomper les taches de vieillesse. Tous les traitements sont topiques, appliqués à la surface de la peau. 

Les pilules contraceptives ont-elles un effet sur les taches de vieillesse ? Les contraceptifs oraux n'ont aucun effet sur les taches de vieillesse. Ils peuvent provoquer un mélasma, une hyperpigmentation de certaines zones du visage, qui peut être confondue avec des taches de vieillesse. 

Après le traitement, les taches de vieillesse réapparaîtront-elles ? Après que les lésions ont été enlevées ou estompées par les différents traitements abordés dans cet article, les mêmes taches ne repousseront pas. Cependant, les changements cutanés endommagés datant de plusieurs années, qui ont conduit à la formation des taches traitées, restent présents. De nouvelles taches de vieillesse peuvent apparaître. 

Les antibiotiques peuvent-ils faire disparaître les taches de vieillesse ? Il n'y a aucune association entre les antibiotiques et les taches de vieillesse, ni pour les causer ni pour les guérir. 

Je suis fumeur. Quel effet cela a-t-il sur les taches de vieillesse ? Le tabagisme affecte négativement chaque cellule du corps. Il endommage la capacité du corps à réparer les cellules abîmées et contribue à l'apparition des taches de vieillesse.

Combien de temps dois-je laisser les agents éclaircissants en vente libre agir avant de consulter un médecin ? Il peut falloir des mois pour voir une amélioration en utilisant des remèdes maison. Un dermatologue certifié peut aider avec des traitements sur ordonnance et au laser. 

Je sais que des dommages ont été causés par les bronzages profonds et l'exposition au soleil quand j'étais beaucoup plus jeune. N'est-il pas trop tard pour protéger ma peau du soleil ? Les dommages peuvent survenir à tout âge, et l'exposition au soleil continue d'augmenter votre risque de former de nouvelles taches de vieillesse et d'autres problèmes cutanés, y compris le risque de cancers de la peau. 

Que puis-je utiliser pour prévenir les taches de vieillesse tout en continuant à utiliser un banc de bronzage ? Il n'existe rien à faire ou à utiliser pour réduire les dommages cutanés tout en continuant à s'exposer aux lampes des bancs de bronzage. 

Je pensais qu'il fallait s'exposer au maximum au soleil pour fournir de la vitamine D à mon corps. Qu'en est-il ? L'exposition au soleil est une bonne source de vitamine D, mais faites-le avec modération et ne vous brûlez pas. La vitamine D se trouve également dans de nombreux aliments, et la vitamine D d'origine alimentaire est supérieure à celle produite par le soleil.
