A psoríase é uma doença autoimune crônica que se desenvolve quando o sistema imunológico da pessoa envia sinais defeituosos que dizem às células da pele para crescerem muito rapidamente. Novas células da pele se formam em dias em vez de semanas. Essas células em excesso não são eliminadas e se acumulam na superfície, causando “placas” ou manchas de psoríase.

A psoríase é a doença autoimune mais comum nos Estados Unidos, afetando 7,5 milhões de pessoas. Pode afetar qualquer parte do corpo. A psoríase está frequentemente ligada a outras condições graves de saúde, como diabetes, doenças cardiovasculares e depressão.

Causas

Cerca de um terço das pessoas com psoríase têm um familiar com a doença. Cientistas acreditam que pelo menos 10% da população herdam um ou mais genes que criam a tendência a desenvolver psoríase, mas apenas dois a três por cento da população desenvolverão a doença.

Acredita-se que para desenvolver psoríase, a pessoa deve ter uma combinação de genes e exposição a gatilhos ambientais. Os gatilhos conhecidos incluem:  

  • Estresse: Pode causar o aparecimento da doença pela primeira vez ou agravar a psoríase existente. Relaxamento e redução do estresse podem ajudar a prevenir que o estresse afete a doença.
  • Lesão ou trauma na pele: Vacinações, queimaduras solares e arranhões são conhecidos por desencadear uma resposta psoriática. O tratamento precoce pode minimizar essa resposta.
  • Medicamentos: Alguns são conhecidos por desencadear psoríase. O lítio, usado para tratar transtorno bipolar e outros distúrbios psiquiátricos, agrava a psoríase em cerca de metade das pessoas com psoríase que o utilizam. Antimaláricos, como cloroquina e hidroxicloroquina, podem causar uma crise de psoríase, geralmente duas a três semanas após o uso do medicamento. Inderal, usado principalmente para pressão alta, piora a psoríase em 25 a 30% das pessoas com psoríase que o tomam. Não se sabe se outros medicamentos da mesma classe (betabloqueadores) têm o mesmo efeito, mas podem ter potencial para uma reação semelhante. Quinidina, um medicamento para o coração, foi relatado como agravante da psoríase. Indometacina, usada para tratar artrite, pode piorar a psoríase. Infecções, especialmente por estreptococos (a bactéria que causa a faringite estreptocócica), podem ser um gatilho. Frequentemente está associada ao primeiro aparecimento da psoríase gutata em crianças. Algumas pessoas com psoríase relatam que alergias, dieta e clima desencadeiam a doença. 
Sinais e Sintomas

Cerca de 95% das vezes, o diagnóstico pode ser feito por inspeção visual, no entanto, às vezes é necessária uma biópsia de pele para um diagnóstico definitivo.

 Existem cinco tipos de psoríase:  

  • A psoríase em placas é caracterizada por manchas vermelhas com um acúmulo prateado-branco de células mortas da pele, aparecendo mais frequentemente no couro cabeludo, joelhos, cotovelos e parte inferior das costas. Frequentemente coçam, doem e podem rachar e sangrar.
  • A psoríase gutata geralmente começa na infância ou em jovens adultos. É o segundo tipo mais comum de psoríase, ocorrendo em cerca de 10 por cento das pessoas que têm psoríase.
  • A psoríase inversa aparece como lesões muito vermelhas nas dobras do corpo. Pode parecer lisa e brilhante. Muitas pessoas têm outro tipo de psoríase no corpo ao mesmo tempo.
  • A psoríase pustulosa é caracterizada por pústulas brancas, bolhas de pus não infeccioso compostas por glóbulos brancos, cercadas por pele vermelha. Não é infecciosa nem contagiosa.
  • A psoríase eritrodérmica é uma forma particularmente inflamatória que frequentemente afeta a maior parte da superfície do corpo. É rara, ocorrendo em três por cento das pessoas com psoríase, frequentemente associada à psoríase em placas instável. As lesões não são claramente definidas. Há vermelhidão intensa e generalizada e descamação da pele, com coceira severa e frequentemente dor. Essa forma de psoríase pode ser fatal.

A psoríase pode ser leve, moderada ou grave. A doença em menos de três por cento do corpo é considerada leve, e constitui cerca de 80 por cento dos casos. De três a 10 por cento do corpo afetado é considerado moderado; mais de 10 por cento do corpo afetado é considerado grave.

Vinte por cento dos casos são moderados a graves. A gravidade também é medida pelo quanto a doença afeta a qualidade de vida da pessoa. A psoríase pode ter um impacto severo nas atividades diárias, mesmo que envolva uma área pequena, como as palmas das mãos ou as solas dos pés. 

Prevenção

A psoríase não é prevenível, mas pode ser útil evitar os gatilhos, se possível. Gatilhos comuns incluem irritação da pele, cortes, tatuagens e certos medicamentos. Infecções como infecções de garganta por estreptococo também podem desencadear a psoríase. Cada vez mais, tem sido demonstrado que escolhas alimentares também podem afetar a psoríase, com alimentos de alto índice glicêmico causando surtos da doença. 

Tratamentos

Em casos leves, como manchas no couro cabeludo, joelhos, cotovelos, mãos e pés, os tratamentos tópicos são suficientes para controlar as placas. Doenças moderadas a graves geralmente envolvem uma combinação de tratamentos: tópicos, fototerapia e medicamentos sistêmicos.

Tópico

 Tratamentos tópicos, medicamentos aplicados na pele, são a primeira linha de defesa no tratamento da psoríase. Eles desaceleram ou normalizam a reprodução excessiva das células da pele e reduzem a inflamação. Alguns estão disponíveis sem receita (OTC), enquanto outros exigem prescrição médica.

Existem dois ingredientes ativos em preparações OTC aprovadas pela FDA para o tratamento da psoríase: ácido salicílico e alcatrão de carvão. O ácido salicílico é um queratolítico, um agente aplicado na pele para amolecer a queratina. Queratolíticos afrouxam e auxiliam na esfoliação das células da pele, fazendo com que as camadas externas da pele se desprendam. Ele amolece e remove as escamas da psoríase. Pode causar irritação se permanecer em contato com a pele por muito tempo.

O alcatrão, derivado do carvão, pode retardar o crescimento rápido das células da pele e restaurar a aparência da pele. Também pode reduzir a inflamação e a coceira.

Existem outros produtos que contêm substâncias como aloe vera, jojoba, piritionato de zinco e capsaicina, usados para hidratar, acalmar, remover escamas ou aliviar a coceira. A eficácia desses agentes não é conhecida. Eles podem causar efeitos colaterais ou reações alérgicas. Se ocorrer irritação, interrompa o uso.

Manter a pele hidratada é importante para reduzir vermelhidão e coceira. Use produtos sem fragrância. Aplique hidratantes após o banho e lavagem das mãos. Use sabonetes hidratantes. Limite banhos mornos a 10 minutos ou menos. Banhos com óleo, aveia, sais de Epsom ou sais do Mar Morto podem remover as escamas e aliviar a coceira. 

Tratamentos tópicos prescritos não esteroides incluem medicamentos derivados da vitamina D. Esteroides tópicos podem ser usados para psoríase leve.

Fototerapia

 A fototerapia envolve expor a pele à luz ultravioleta regularmente, sob supervisão médica. Os tratamentos são realizados no consultório do médico, em clínicas de psoríase ou em casa com uma unidade de fototerapia. A chave para o sucesso é a consistência.

A Fundação Nacional de Psoríase não apoia o uso de camas de bronzeamento internas como substituto para a fototerapia supervisionada por médicos. A Academia Americana de Dermatologia, a FDA e os Centros de Controle e Prevenção de Doenças (CDC) desencorajam o uso de camas de bronzeamento e lâmpadas solares.

Os tratamentos com luz ultravioleta B (UVB)/luz ultravioleta A (UVA) penetram na pele e retardam o crescimento das células da pele afetadas. A psoríase pode piorar temporariamente antes de melhorar. Ocasionalmente, ocorrem surtos temporários com a terapia de luz UV. Pode ser combinada com tratamentos tópicos ou sistêmicos, aumentando dramaticamente a eficácia de outros agentes sistêmicos.

O laser excimer, recentemente aprovado pela FDA para o tratamento da psoríase crônica localizada, emite um feixe de alta intensidade de UVB. O laser de corante pulsado é aprovado para o mesmo uso. Ele destrói os pequenos vasos sanguíneos que contribuem para a formação das placas psoriáticas.

Medicamentos Sistêmicos

Medicamentos sistêmicos são tomados por via oral em forma líquida ou de comprimidos, ou administrados por injeção. Eles incluem:

● Acitretina é uma forma sintética de vitamina A que desacelera a rápida renovação celular observada na pele psoriática.
● Ciclosporina suprime o sistema imunológico. Foi usada inicialmente para ajudar a prevenir rejeição em pacientes de transplante de órgãos. A FDA aprovou seu uso para adultos com psoríase grave.
● Metotrexato foi inicialmente usado para tratar câncer. Ele inibe uma enzima envolvida no rápido crescimento das células da pele. Seu uso deve ser monitorado muito de perto; possui muitos avisos sobre efeitos adversos e colaterais.

    Medicamentos Biológicos

    Os medicamentos biológicos, também chamados de "biológicos", são usados para psoríase moderada a grave. São administrados por injeção ou infusão intravenosa (IV), direcionando partes específicas do sistema imunológico. Os biológicos atuam bloqueando a ação de um tipo específico de célula imune, chamadas células T, ou bloqueando proteínas do sistema imunológico que desencadeiam a inflamação no corpo. Alguns dos biológicos são: Enbrel, Humira, Remicade, Simponi e Stelara. Existem riscos e efeitos colaterais significativos.

    • A Terapia de Spa é o uso de águas minerais. Acredita-se que ajuda a pele a reter a hidratação e a amaciar a pele, quebrando as placas que caracterizam a psoríase.

    Vivendo com Psoríase

     

    O estresse pode fazer com que o sistema imunológico libere substâncias químicas que causam inflamação. Em pessoas com psoríase, o sistema imunológico reage em excesso, liberando muitas dessas substâncias. A pessoa com psoríase precisa minimizar o estresse físico e mental.

    A coceira da psoríase pode ter um impacto maior na qualidade de vida do que as lesões visíveis. É uma coceira única, frequentemente descrita como uma sensação de queimação ou mordida. Alguns a descrevem como a sensação de serem picados por formigas de fogo.

    Remédios caseiros para a coceira incluem manter a pele hidratada. Isso reduz a inflamação, a coceira e ajuda a pele a cicatrizar. Minimizar a descamação e a formação de crostas é útil. Banhos frios e compressas frias podem ajudar a aliviar a coceira.

    A psoríase pode afetar os relacionamentos de várias maneiras. Pode ser difícil falar com a família e amigos sobre a doença. Namorar pode ser evitado devido à má imagem corporal. A psoríase genital pode ter um impacto significativo no sexo e na intimidade. A depressão é a doença mais comum associada à psoríase; é duas vezes mais comum do que na população geral. Pode ter um impacto significativo na qualidade de vida.

     

     

    Pessoas com psoríase têm maior probabilidade de ter pensamentos e tentativas de suicídio. Uma pessoa com sintomas de depressão deve ser avaliada por um médico. Os sintomas incluem:

    ● Incapacidade de dormir
    ● Incapacidade de sair da cama
    ● Perda de energia
    ● Falta de interesse em coisas que antes gostava
    ● Incapacidade de se concentrar

    Trabalhar com psoríase e artrite psoriática pode ser desafiador. Reunir-se com um supervisor para discutir como a psoríase pode afetar o desempenho no trabalho é útil. O ambiente de trabalho deve ser confortável. Pode ser necessário inventar maneiras de realizar tarefas rotineiras. A área de trabalho e os horários devem ser mantidos organizados.
