Lentigos solares é o termo médico para manchas senis, às vezes chamadas de manchas de fígado. Ocorrem mais frequentemente na pele que teve maior exposição ao sol: rosto, pescoço, mãos, braços, ombros e parte superior das costas. Manchas senis geralmente são planas e de cor marrom, preta ou cinza. São comuns após os 40 anos, mas podem ocorrer em pessoas mais jovens. Algumas podem ter aparência semelhante a uma condição médica mais séria. 

Manchas senis são inofensivas, mas podem causar preocupação quanto ao câncer. Lesões que aparecem espontaneamente, mudam de forma ou crescem rapidamente devem ser verificadas quanto a bordas irregulares e pigmentos variados dentro da mesma lesão.

Como Clarear Manchas Senis

Por razões cosméticas, manchas senis podem ser clareadas com agentes despigmentantes ou removidas cirurgicamente. O melhor manejo das manchas senis é preveni-las usando protetor solar e evitando a exposição direta ao sol o máximo possível. Escolha protetores solares naturais que não contenham parabenos ou petroquímicos.  

Causas

Manchas senis são causadas principalmente por anos de exposição à luz ultravioleta (UV) do sol. Lâmpadas e camas de bronzeamento comercial também danificam a pele. A melanina é o pigmento na camada superior da pele (epiderme), que dá à pele sua cor normal. A luz UV acelera a produção de melanina, causando bronzeamento, que protege as camadas mais profundas da pele da luz UV.

Manchas senis se formam quando a melanina "se aglomera" ou está, em algumas áreas, em concentração particularmente alta. O envelhecimento também pode acelerar a produção de melanina, mesmo sem exposição ao sol. Pode haver algum efeito genético, com algumas pessoas mais suscetíveis do que outras a formar manchas senis. Pessoas de pele clara são mais propensas a formar manchas senis do que pessoas com pele naturalmente mais escura. Também há risco aumentado com histórico de exposição solar excepcionalmente intensa e frequente. (1,2)

Lentigos solares são: 

  • macular - uma área plana e demarcada de aumento da pigmentação da pele que não é elevada nem deprimida.
  • geralmente marrons, pretas ou cinzas.
  • encontradas na pele que teve maior exposição ao sol ao longo dos anos, geralmente nas costas das mãos, parte superior dos pés, rosto, ombros e parte superior das costas.
  • variável em tamanho, de 1 milímetro a 3 centímetros de diâmetro.
  • tendem a se agrupar, tornando-as mais evidentes.
  • às vezes semelhante em aparência a lesões malignas ou não malignas.
  • mais comum em pessoas brancas ou asiáticas, especialmente em indivíduos com tendência a ter sardas.

Melasma e efélides são dois tipos de lesões maculares hiperpigmentadas não malignas que podem ser confundidas com manchas senis. O melasma é uma hiperpigmentação progressiva e não descamativa da pele exposta ao sol, especialmente no rosto e antebraços.

A formação do melasma está frequentemente associada à gravidez, contraceptivos orais e alguns anticonvulsivantes. Pode surgir espontaneamente, sem causa identificável ou associação a fatores coexistentes conhecidos. Ocorre em mulheres nove vezes mais frequentemente do que em homens, e mais frequentemente em tipos de pele comuns na Ásia, Oriente Médio e América do Sul. Não traz consequências para a saúde, mas pode ser desconfortável do ponto de vista estético. 

O rosto é o local mais comum do melasma, geralmente no centro da face; menos frequentemente ocorre ao redor da boca e mandíbula. As manchas geralmente são bilaterais. Existem três tipos de melasma: epidérmico, dérmico e misto. O melasma epidérmico é geralmente marrom claro e se destaca sob exame com lâmpada de Wood. O melasma dérmico é acinzentado e não se destaca com a lâmpada de Wood. Lesões mistas são marrom escuro com resposta variável à lâmpada de Wood. Epélides é o nome médico para sardas, geralmente de 1 a 2 milímetros, bem definidas, no rosto, pescoço, tórax e braços. (2) 

Lentiginoses solares podem se assemelhar a pintas (nevos), que podem ser elevadas ou planas, e não se limitam às áreas expostas ao sol. Ceratoses seborréicas, que são benignas, são pequenas, de cor bege, marrom ou preta, com aparência de "coladas". Há uma ampla variação de tamanho normal, de 1 milímetro a mais de 2,5 centímetros. Lentigo maligno é um tipo de câncer de pele, um melanoma, que pode se desenvolver em áreas de exposição solar prolongada. Começa como bege a marrom a preto, escurece e aumenta de tamanho. Apresenta bordas irregulares e múltiplas pigmentações na mesma lesão. Ceratoses actínicas pigmentadas são pré-malignas. (2,4) 

Novas lesões cutâneas devem sempre ser avaliadas por um dermatologista certificado. As seguintes características são preocupantes:

  • pigmentação escura
  • crescimento rápido
  • bordas irregulares
  • múltiplas cores na mesma lesão
  • sensibilidade ou sangramento
  • pele com coceira e vermelhidão
  • dor
  • cicatrização deficiente

Qualquer lesão atípica ou sugestiva de melanoma deve ser biopsiada, abrangendo todas as camadas da pele. Dermatoses seborréicas e ceratoses actínicas pigmentadas nem sempre podem ser distinguidas apenas pela visualização. Em caso de dúvida diagnóstica, deve-se realizar a biópsia. (1,2,4) 

Alguns distúrbios sistêmicos se manifestam inicialmente como múltiplas lentiginoses: síndrome de Peutz-Jeghers, síndrome LEOPARD e síndrome de Lamb.    

Prevenção

Cuidados com o corpo  
  • Recomenda-se evitar a luz solar direta entre 10h e 16h, quando os raios solares são mais intensos. As atividades ao ar livre devem ser programadas antes das 10h ou após as 16h.
  • Utilizar protetor solar não apenas no rosto, mas em locais comumente esquecidos, como o dorso das mãos, pode ajudar na prevenção das manchas senis.
  • Protetor solar, que protege contra os raios UVA e UVB, com fator de proteção solar de pelo menos 15, deve ser aplicado 15-30 minutos antes da exposição ao sol e reaplicado a cada duas horas. Recomenda-se o uso de protetor solar de amplo espectro com fator de proteção de pelo menos 30.
  • Deve ser reaplicado com mais frequência ao nadar ou suar. Não deve conter ingredientes potencialmente nocivos, como parabenos ou petroquímicos. Para mais informações, veja nosso relatório gratuito, "Os Veneno Que Você Passa no Rosto Todos os Dias."
  • Cubra-se. Use um chapéu de aba larga, que oferece mais proteção do que um boné ou viseira. Use roupas de tecido fechado, cobrindo braços e pernas. Roupas projetadas para proteção solar são úteis. Um fator de proteção ultravioleta (UPF) de 40-50 nas roupas é a melhor proteção. O tecido pode perder a proteção ao esticar, molhar ou com lavagens repetidas. (2)

  Alimente suas células da pele diariamente com os nutrientes que elas precisam para se manterem hidratadas e radiantes. Embora as manchas senis não representem ameaça à saúde, podem ser uma preocupação estética. Existem tratamentos disponíveis para clareá-las ou removê-las. A pigmentação está na parte mais profunda da epiderme, a camada superior da pele. Qualquer tratamento para clarear as manchas deve penetrar até a parte mais profunda da epiderme. Os tratamentos são considerados estéticos, portanto, geralmente o seguro não cobre. Deve-se escolher um médico treinado e experiente nos diversos tratamentos em consideração.  

  • Medicamentos. Cremes clareadores prescritos (hidroquinona) usados sozinhos ou com retinoides (tretinoína) e um esteroide suave podem clarear gradualmente as manchas senis ao longo de meses. É fortemente recomendado o uso de protetor solar natural com FPS 30 (sem parabenos) durante o tratamento medicamentoso. Pode causar vermelhidão, queimação, ressecamento ou coceira.
  • Terapia a laser. A terapia a laser destrói os melanócitos, as células produtoras de pigmento, sem danificar a superfície da pele. Normalmente são necessárias várias sessões. Após o tratamento, as manchas senis desbotam gradualmente, ao longo de semanas ou meses. Existem poucos efeitos colaterais, mas pode haver leve descoloração da pele.
  • Crioterapia (congelamento). Nitrogênio líquido ou outro agente congelante é usado para destruir o pigmento excessivo. Geralmente é usado em uma única mancha senil ou pequenos grupos de manchas. Pode irritar a pele e há um pequeno risco de cicatrizes permanentes ou descoloração.
  • Dermabrasão. Este tratamento consiste em aplainar a superfície da pele usando uma escova que gira rapidamente; a pele nova substitui a antiga. Pode haver vermelhidão temporária e formação de crostas.
  • Peeling químico. Este método de tratamento envolve a aplicação de ácido na pele, queimando a camada externa da pele. À medida que a pele descama, uma nova pele se forma. Geralmente são necessários vários tratamentos para ver resultados. A proteção solar é fortemente recomendada após o tratamento com peelings químicos. Há irritação temporária e um leve risco de descoloração.
  • Ácido natural. O uso diário de um ácido eficaz e natural, como o ácido lático (que vem do leite), pode ajudar a tratar a hiperpigmentação. Pode ser usado diariamente em um regime de cuidados domiciliares em porcentagens mais baixas, ou em porcentagens mais altas em uma peel químico mensal. No entanto, os ácidos devem ser usados apenas sob os cuidados de um profissional licenciado em cuidados com a pele, como esteticista ou dermatologista. Os efeitos colaterais incluem vermelhidão, algum desconforto/sensação de queimação, ressecamento e maior sensibilidade ao sol.

Estilo de Vida e Remédios Caseiros

Cremes e loções clareadoras estão disponíveis sem receita em lojas de departamento, farmácias e também na internet. Eles podem melhorar a aparência das manchas senis ao longo de meses. Os resultados dependem um pouco do quão pigmentadas estão as manchas e da frequência de aplicação do creme ou loção. O produto usado pode conter hidroquinona, deoxiarbutina, ácido glicólico ou ácido kójico, que podem causar irritação na pele. (4,2)
