Definicja
Trądzik różowaty to powszechna przewlekła choroba skóry powodująca zaczerwienienie twarzy. Zaczerwienienie często towarzyszy małym, czerwonym grudkom wypełnionym ropą, charakteryzującym się zaostrzeniami i remisjami. Zwykle zaczyna się po 30. roku życia jako rumień na policzkach, nosie, brodzie lub czole, który nasila się i ustępuje. W niektórych przypadkach trądzik różowaty może również występować na szyi, klatce piersiowej, skórze głowy lub uszach. Z czasem rumień staje się bardziej trwały i intensywny. Mogą pojawić się widoczne naczynia krwionośne. Jeśli nie jest leczony, często rozwijają się grudki i krosty. W niektórych ciężkich przypadkach nos może stać się opuchnięty i guzowaty z powodu nadmiaru tkanki. Ten stan nazywa się rinofimą. U wielu pacjentów dotknięte są również oczy, które są podrażnione i wyglądają na łzawiące lub przekrwione.

Trądzik różowaty może dotknąć każdego, ale największe ryzyko mają osoby o jasnej karnacji, które łatwo się rumienią. Choroba częściej dotyka kobiety niż mężczyzn, ale najcięższe objawy występują u mężczyzn, prawdopodobnie dlatego, że zwlekają z leczeniem aż do zaawansowanego stadium choroby.

Nie ma lekarstwa na trądzik różowaty, a jego przyczyna jest nieznana, ale terapia medyczna może kontrolować lub odwracać objawy. Osoby podejrzewające u siebie trądzik różowaty powinny udać się do dermatologa lub innego doświadczonego lekarza w celu diagnozy i odpowiedniego leczenia.

 Objawy i symptomy

Objawy trądziku różowatego mogą obejmować:

  • Rumień twarzy: Trądzik różowaty zwykle powoduje utrzymujący się rumień w centralnej części twarzy. Małe naczynia krwionośne na nosie i policzkach często puchną i stają się widoczne.
  • Opuchnięte czerwone grudki: Wiele osób z trądzikiem różowatym rozwija na twarzy grudki przypominające trądzik. Czasami zawierają one ropę. Skóra może być bolesna i gorąca w dotyku. 
  • Problemy z oczami: Około połowa osób z trądzikiem różowatym doświadcza suchości oczu, podrażnień oraz opuchniętych, zaczerwienionych powiek. U niektórych osób objawy ze strony oczu pojawiają się przed objawami skórnymi.
  • Powiększony nos: Rzadko trądzik różowaty może pogrubiać skórę nosa, powodując, że nos wydaje się bulwiasty (rinofima). Częściej występuje to u mężczyzn niż u kobiet.

Przyczyny i czynniki wywołujące

 

Przyczyna trądziku różowatego jest nieznana, ale uważa się, że jest to kombinacja czynników dziedzicznych i środowiskowych.

Istnieją czynniki, które mogą wywołać lub nasilić trądzik różowaty poprzez zwiększenie przepływu krwi do powierzchni skóry. Niektóre z tych czynników to: 

  • Gorące potrawy lub napoje
  • Ostre potrawy
  • Alkohol
  • Ekstremalne temperatury
  • Światło słoneczne
  • Stres, złość lub zażenowanie
  • Intensywne ćwiczenia
  • Gorące kąpiele lub sauny
  • Kortykosteroidy, takie jak prednizon
  • Leki rozszerzające naczynia krwionośne, w tym niektóre leki na nadciśnienie

Rozpoznanie
Rozpoznanie stawia się na podstawie badania skóry, uwzględniając objawy i zmienność ich nasilenia w czasie. Nie ma specyficznego testu na trądzik różowaty.

Leczenie

Chociaż nie ma lekarstwa na trądzik różowaty, leczenie może kontrolować i zmniejszać objawy. Najlepszy możliwy efekt zwykle wymaga połączenia leków na receptę i zmian stylu życia. Procedury chirurgiczne i inne, a także leki na receptę stosowane w trądziku różowatym, mogą obejmować:

  • Antybiotyki. Antybiotyki stosowane w leczeniu trądziku różowatego mają działanie przeciwzapalne. Istnieją różne metody podawania, w tym kremy, żele lub balsamy do stosowania miejscowego oraz tabletki doustne. Tabletki antybiotykowe zwykle są skuteczniejsze przy krótkotrwałym stosowaniu, ale mogą mieć więcej skutków ubocznych.
  • Leki na trądzik. Jeśli antybiotyki nie działają, izotretynoina (Accutane, Claravis i inne) może dać lepszy efekt. To silny lek zwykle zarezerwowany dla ciężkiego trądziku torbielowatego. Jest ściśle regulowany, zwłaszcza w leczeniu kobiet w wieku rozrodczym. Jest znany z powodowania poważnych wad wrodzonych. Te leki mogą wiązać się z poważnym ryzykiem; koniecznie porozmawiaj z lekarzem o potencjalnych zagrożeniach i skutkach ubocznych.
  • Chirurgia laserowa i elektrochirurgia. Mogą zmniejszyć widoczność naczyń krwionośnych i usunąć nagromadzenie tkanek wokół nosa, poprawiając wygląd.

Najlepszym krokiem do zdrowszej skóry jest unikanie potencjalnie szkodliwych chemikaliów i stosowanie w zamian 100% czystych składników. 

Alternatywne i naturalne leczenie

To najczęściej stosowane alternatywne metody leczenia:

  • Krem Chrysanthellum Indicum: wykorzystuje ekstrakt z zioła o tej samej nazwie. Dowody naukowe są ograniczone, ale reakcje niepożądane opisuje się jako łagodne i nie większe niż reakcje na placebo. Jedno badanie wykazało, że lek jest znacznie skuteczniejszy niż placebo.
  • Krem z niacynamidem, forma witaminy B3, okazał się skutecznym nawilżaczem, ale dane są skąpe.
  • Krem z kwasem azelainowym, pochodzący z pszenicy, żyta i jęczmienia, uważa się, że łagodzi zaczerwienienia i zmniejsza grudki oraz krosty.

Po osiągnięciu remisji leczenie można utrzymać, identyfikując i unikając czynników stylu życia i środowiskowych, które wywołują zaostrzenia lub pogarszają stan. Odnieś się do sekcji z listą czynników wywołujących, aby dowiedzieć się, czego unikać. Propozycje zapobiegania zaostrzeniom obejmują:

  • Stosuj krem z filtrem o szerokim spektrum działania i SPF 30 lub wyższym.
