Definitie
Rosacea is een veelvoorkomende chronische huidaandoening die roodheid in het gezicht veroorzaakt. De roodheid gaat vaak gepaard met kleine, rode, met pus gevulde papels, gekenmerkt door opvlammingen en remissies. Het begint meestal na het 30e levensjaar, als roodheid op de wangen, neus, kin of voorhoofd, die wisselt in intensiteit. In sommige gevallen kan rosacea ook voorkomen op de nek, borst, hoofdhuid of oren. Na verloop van tijd wordt de roodheid aanhoudender en roder. Zichtbare bloedvaatjes kunnen verschijnen. Als het onbehandeld blijft, ontwikkelen zich vaak bulten en puistjes. In sommige ernstige gevallen kan de neus gezwollen en bobbelig worden door overtollig weefsel. Deze aandoening heet rhinophyma. Bij veel patiënten zijn ook de ogen aangetast, die geïrriteerd aanvoelen en waterig of bloeddoorlopen lijken.

Rosacea kan iedereen treffen, maar mensen met een lichte huid die gemakkelijk blozen lopen het grootste risico. Het komt vaker voor bij vrouwen dan bij mannen, maar de ernstigste tekenen en symptomen worden bij mannen gevonden, mogelijk omdat zij behandeling uitstellen tot de ziekte gevorderd is.

Er is geen genezing voor rosacea en de oorzaak is onbekend, maar medische behandeling kan de tekenen en symptomen beheersen of omkeren. Personen die vermoeden rosacea te hebben, worden aangeraden een dermatoloog of andere deskundige arts te raadplegen voor diagnose en passende behandeling.

 Tekenen en symptomen

Tekenen en symptomen van rosacea kunnen zijn:

  • Roodheid van het gezicht: Rosacea veroorzaakt meestal een aanhoudende roodheid in het centrale deel van het gezicht. Kleine bloedvaatjes op de neus en wangen zwellen vaak op en worden zichtbaar.
  • Gezwollen rode bulten: Veel mensen met rosacea ontwikkelen bulten op hun gezicht die op acne lijken. Soms bevatten ze pus. De huid kan gevoelig en warm aanvoelen. 
  • Oogproblemen: Ongeveer de helft van de mensen met rosacea ervaart ook droge ogen, irritatie en gezwollen, roodachtige oogleden. Bij sommige mensen treden oogklachten op vóór de huidklachten.
  • Vergrote neus: Zelden kan rosacea de huid op de neus verdikken, waardoor de neus bolvormig lijkt (rhinophyma). Dit komt vaker voor bij mannen dan bij vrouwen.

Oorzaken en uitlokkers

 

De oorzaak van rosacea is onbekend, maar men denkt dat het een combinatie is van erfelijke en omgevingsfactoren.

Er zijn enkele factoren die rosacea kunnen uitlokken of verergeren door de bloedtoevoer naar het oppervlak van uw huid te verhogen. Enkele van deze factoren zijn: 

  • Heet voedsel of dranken
  • Pittig voedsel
  • Alcohol
  • Temperatuurextremen
  • Zonlicht
  • Stress, woede of schaamte
  • Zware inspanning
  • Warme baden of sauna’s
  • Corticosteroïden, zoals prednison
  • Medicijnen die bloedvaten verwijden, waaronder sommige bloeddrukmedicijnen

Diagnose
De diagnose wordt gesteld door onderzoek van de huid, met inachtneming van symptomen en fluctuatie van tekenen en symptomen in de tijd. Er is geen specifieke test voor rosacea.

Behandeling

Hoewel er geen genezing is voor rosacea, kunnen behandelingen de tekenen en symptomen beheersen en verminderen. Het beste resultaat vereist meestal een combinatie van voorgeschreven behandelingen en veranderingen in levensstijl. Chirurgische en andere procedures, evenals voorgeschreven medicijnen voor rosacea, kunnen omvatten:

  • Antibiotica. De antibiotica die voor rosacea worden gebruikt, hebben ontstekingsremmende effecten. Er zijn verschillende toedieningsmethoden, waaronder crèmes, gels of lotions voor uitwendig gebruik, en orale pillen. Antibiotica in pilvorm zijn meestal effectiever voor kortdurend gebruik, maar kunnen meer bijwerkingen hebben.
  • Acnemedicatie. Als antibiotica niet effectief zijn, kan isotretinoïne (Accutane, Claravis, anderen) een beter resultaat geven. Het is een krachtig medicijn dat meestal wordt gereserveerd voor ernstige cystische acne. Het is streng gereguleerd, vooral voor de behandeling van vrouwen in hun vruchtbare jaren. Het staat bekend om ernstige geboorteafwijkingen te veroorzaken. Deze medicijnen kunnen grote risico’s hebben; overleg zeker met uw zorgverlener over mogelijke gevaren en bijwerkingen.
  • Laserchirurgie en elektrochirurgie. Deze kunnen de zichtbaarheid van bloedvaten verminderen en weefselophoping rond de neus verwijderen, wat het uiterlijk verbetert.

De beste stap naar een gezondere huid begint met het vermijden van potentieel schadelijke chemicaliën en het gebruik van 100% pure ingrediënten in plaats daarvan. 

Alternatieve en Natuurlijke Behandeling

Dit zijn de meest gebruikte alternatieve behandelingen:

  • Chrysanthellum Indicum Crème: gebruikt een extract van het kruid met dezelfde naam. Onderzoeksbewijzen zijn beperkt, maar bijwerkingen worden als mild beschreven en niet erger dan reacties op placebomedicatie. Eén studie toonde aan dat het medicijn significant effectiever is dan placebomedicatie.
  • Niacinamidecrème, een vorm van vitamine B3, is gevonden als een effectieve moisturizer, maar de gegevens zijn schaars.
  • Azelaïnezuurcrème, afgeleid van tarwe, rogge en gerst, wordt verondersteld roodheid te verminderen en papels en pustels te verminderen.

Zodra de behandeling remissie bereikt, kan deze worden onderhouden door leefstijl- en omgevingsfactoren te identificeren en te vermijden die opvlammingen veroorzaken of de aandoening verergeren. Raadpleeg de sectie met triggers om te zien wat u mogelijk moet vermijden. Suggesties om opvlammingen te voorkomen zijn onder andere:

  • Draag een breedspectrum zonnebrandcrème met een SPF van 30 of hoger.
  • Bescherm uw gezicht bij koud weer – gebruik een sjaal of skimasker.
  • Vermijd het irriteren van uw gezichtshuid door er te veel aan te wrijven of te veel aan te raken.
  • Was probleemgebieden met een milde reiniger (Dove, Cetaphil).
  • Vermijd gezichtsproducten die alcohol of andere huidirriterende stoffen bevatten. Veel huidverzorgingsproducten bevatten bovendien mogelijk schadelijke ingrediënten.
  • Gebruik schone en eenvoudige ingrediënten om uw huid dagelijks te hydrateren en te revitaliseren.

Uw vragen beantwoord

  1. Kan ik make-up dragen?  
    Het is veilig om make-up te dragen als u rosacea heeft. Overweeg groene foundations en poeders te gebruiken, omdat deze ontworpen zijn om roodheid tegen te gaan.
  2. Helpen vitamines?
    Vitamine K en A, oraal ingenomen, zouden de symptomen verbeteren. Er is geen sluitend bewijs hiervoor en het innemen van te veel vitamines kan gevaarlijk zijn.
  3. Zijn er voedingssupplementen die helpen?  
    Bespreek mogelijke supplementen met uw arts voordat u ze gebruikt. Fabrikanten doen vaak onjuiste claims.
  4. Kan ik naar het strand gaan?  
    Ja, met voorzorgsmaatregelen. Gebruik royaal een natuurlijke zonnebrandcrème (zonder parabenen of petrochemicaliën) en breng deze minstens elke 2 uur opnieuw aan. Draag een hoed met brede rand en gebruik een parasol tegen de zon.
  5. Mijn rosacea maakt me depressief. Wat kan ik doen?  
    Bespreek de depressie met uw arts. Er zijn behandelingen voor depressie.
  6. Ik voel me zo alleen hiermee. Wat kan ik doen?  
    Een rosacea steungroep kan u in contact brengen met anderen die dezelfde problemen ervaren. U kunt de National Rosacea Society contacteren voor groepen bij u in de buurt of online zoeken naar groepen. (Contactgegevens hieronder.)
  7. Ik had als tiener ernstige acné. Betekent dat dat ik rosacea zal krijgen?  
    Er is geen bewijs dat tieneracné wijst op toekomstige rosacea.
  8. Beïnvloeden voedingsmiddelen rosacea?
    Pittig eten kan een opvlamming veroorzaken. Leer welke voedingsmiddelen bij u een reactie uitlokken en vermijd deze. Omdat rosacea een ontstekingsproces is, kunnen ontstekingsremmende voedingsmiddelen nuttig zijn. Neem contact op met de National Rosacea Society of uw arts voor informatie over voedingsmiddelen die voor u gunstig kunnen zijn.
  9. Ik schaam me en wil het huis niet uit. Wat moet ik doen?
    Vraag uw arts om een verwijzing naar een counselor, die u kan helpen leren omgaan met rosacea. U kunt een vervullend leven hebben.
