Cosa sono i foruncoli gengivali


Come appaiono i foruncoli gengivaliI foruncoli gengivali - noti anche come parulo - sono punti di drenaggio per ascessi alle radici dei denti. Quando il nervo di un dente muore, l’ascesso uscirà dal dente vicino alla sua radice. In risposta a ciò, il corpo invia globuli bianchi per distruggere l’infezione e, quando questi globuli bianchi muoiono, formano un ascesso. Se non trattato, l’ascesso romperà i tessuti della bocca e si drenerà. Questo punto di drenaggio è noto come parulo o foruncolo gengivale.





I foruncoli gengivali possono essere pieni di liquido o costituiti da tessuto gengivale fibroso solido. Il colore può apparire normale oppure l’area del foruncolo gengivale può apparire rossa e irritata.


 

Tipi di foruncoli gengivali

Cosa causa i foruncoli gengivali? I foruncoli gengivali hanno cause e variazioni diverse. Possono anche essere chiamati tumori. Conoscere cosa causa i foruncoli gengivali nella tua bocca può aiutarti a trattarli in modo appropriato.

  • L’ipertrofia gengivale può essere dovuta a un effetto collaterale di farmaci prescritti, protesi dentarie mal adattate o all’uso di apparecchi ortodontici. Questi sono probabilmente la forma più comune di foruncoli gengivali.

  • Una forma comune di tumore gengivale è il “tumore della gravidanza” sulle gengive, indotto dai livelli ormonali durante la gravidanza. Questi scompaiono rapidamente dopo il termine della gravidanza e il ritorno alla normalità dei livelli ormonali.

  • Foruncoli gengivali congeniti possono essere presenti alla nascita.

  • Ascessi dentali dovuti a carie gravi e infezione del nervo del dente possono apparire come un foruncolo sulle gengive.

  • I foruncoli causati da grave malattia parodontale sono presenti quando l'igiene orale e il controllo della placca sono scarsi.

Segni e Sintomi dei Foruncoli Gengivali

I foruncoli gengivali in bocca possono apparire in diverse forme:
  • Aree gonfie e dure di tessuto fibroso
  • Vesciche morbide, gonfie e piene di liquido
  • Rosa chiaro
  • Rosso scuro
  • Nessun sanguinamento, mancanza di apporto sanguigno
  • Sanguinamento durante l'igiene orale

Anche se non si trovano effettivamente dentro o sopra un dente, i foruncoli gengivali di solito causano forti mal di denti. Questo perché l'infiammazione preme sul nervo di altri denti. Altri sintomi che possono verificarsi sono nausea, febbre, arrossamento in bocca o sul viso, alito cattivo, diarrea, ghiandole gonfie, sensibilità al caldo o al freddo, dolore al tatto o durante la masticazione.

Pericoli e Rischi per la Salute

I foruncoli gengivali possono essere completamente benigni oppure segnalare un'infezione grave nella bocca. La maggior parte dei foruncoli gengivali benigni non presenta sintomi come dolore o sanguinamento, mentre quelli con sintomi tendono a essere associati a infezioni dentali gravi.







Se non trattate, infezioni come quelle associate agli ascessi dentali possono diffondersi nel corpo e aumentare i rischi di malattie cardiovascolari, altre malattie sistemiche o, in rari casi, ascessi cerebrali. 1


Se un foruncolo gengivale o un ascesso non vengono trattati, l'infezione può diffondersi ad altri denti o addirittura ad altre parti del corpo. Il gonfiore può aumentare e in alcuni casi ostruire le vie aeree causando la morte. Pericoli più immediati includono la perdita dei denti a causa dell'erosione ossea provocata dall'infezione.

 

Trattamenti per i Foruncoli Gengivali

Non solo sarà necessario drenare l'ascesso, ma probabilmente avrai bisogno di antibiotici per combattere l'infezione residua. Tuttavia, questo tratta solo la superficie della situazione. In molti casi, poiché il foruncolo gengivale è solo il punto di drenaggio e un indicatore, potresti aver bisogno anche di una devitalizzazione del dente in necrosi. Se l'area non migliora o peggiora dopo un'igiene domiciliare dedicata, potrebbe essere necessario consultare un professionista per il trattamento.



 

I foruncoli gengivali fanno male? La maggior parte dei foruncoli gengivali di solito non fa male. Se il foruncolo è dovuto a un'infezione ed è un ascesso dentale, potrebbe esserci dolore fino a quando non si può eseguire una terapia canalare.



 

Posso guarire i foruncoli gengivali da solo? A seconda del tipo di infezione che causa il foruncolo gengivale, potresti essere in grado di guarirlo da solo a casa. Quando hai un dente ascessato, la fistola visibile (foruncolo gengivale) tende ad andare e venire. Quindi, anche se la rimuovi facendola drenare, c'è ancora un'infezione sottostante.



Dove possono comparire – lingua, labbro, palato? Un foruncolo gengivale di solito si riferisce al brufolo visibile o alla fistola causata da un ascesso dentale. Queste lesioni si verificano da qualche parte lungo le radici dei denti. Ma poiché alcune radici sono piuttosto lunghe, è possibile vederne uno sul palato. Il gonfiore su labbra o lingua sarebbe dovuto a un altro tipo di infezione dentale.



I foruncoli gengivali sono comuni? Sì. Ogni volta che hai un ascesso o un dente in necrosi, c'è un'alta probabilità che tu veda un foruncolo gengivale (fistola o brufolo) a un certo punto. Tuttavia, l'assenza di un foruncolo gengivale non significa che il dente non sia infetto. Potresti anche vedere un foruncolo gengivale se hai una malattia gengivale aggressiva.



I foruncoli gengivali scompariranno da soli? La maggior parte dei foruncoli gengivali o delle fistole dentali tende a seguire un ciclo di gonfiore e drenaggio. Potresti vederne uno oggi, ma non domani. Sfortunatamente, la causa dell'infezione non scomparirà da sola. Deve essere trattata.

All'interno dei foruncoli gengivali c'è una combinazione di diversi batteri anaerobi gram-negativi e gram-positivi batteri, che causano infiammazione e formazione di pus. In alcuni casi, l'estensione dei batteri e la diffusione del pus possono portare a sepsi e richiedere il ricovero ospedaliero.



I foruncoli gengivali possono farti ammalare? Se sono abbastanza gravi, sì. I foruncoli gengivali cronici e non trattati possono portare a sepsi potenzialmente letale che richiede ricovero e trattamento medico (soprattutto se l'infezione colpisce un bambino). Se l'ascesso dentale inizia a diffondersi al viso, cerca immediatamente assistenza medica.



Cosa succede se un bambino ha un foruncolo gengivale? I foruncoli gengivali possono essere pericolosi per la vita dei bambini, a causa della vicinanza al cervello. Un ascesso dentale pediatrico non trattato deve essere curato appena diagnosticato. Non è raro che i bambini necessitino di ricovero ospedaliero a causa di un foruncolo gengivale non trattato.



I foruncoli gengivali sono cancerosi? Di solito no. Tuttavia, qualsiasi lesione orale che non guarisce entro 10-14 giorni dovrebbe essere valutata dal dentista come parte di uno screening per il cancro orale. Le piaghe precancerose o sospette potrebbero necessitare di una biopsia. Poiché il cancro orale è spesso difficile da rilevare, i primi segni o sintomi potrebbero indicare che la malattia ha già raggiunto uno stadio aggressivo.



Possono cani e gatti avere foruncoli gengivali? Sì. Qualsiasi mammifero, incluso il tuo animale domestico, può sviluppare un foruncolo gengivale. I foruncoli sono semplicemente il risultato di un dente infetto o in necrosi. Se il tuo animale ha avuto una lotta con un altro e si è ferito un dente, il trauma potrebbe essere sufficiente a scatenare un foruncolo gengivale anche se sono passati mesi.

Come trattano i dentisti le bolle gengivali? La maggior parte delle bolle gengivali è dovuta ad ascessi dentali. I dentisti trattano i denti ascessi con antibiotici, terapia canalare o estrazioni dentali. Se la bolla è causata da malattia gengivale, potrebbe essere consigliata una pulizia profonda.



L'acqua salata è utile per le bolle gengivali? Sì. Poiché le bolle gengivali sono causate da infiammazione e gonfiore, sciacquare con acqua tiepida salata può aiutare a ridurre l'entità e la dimensione della fistola nella bocca. La soluzione aiuta a drenare il gonfiore dai tessuti circostanti.



Le bolle gengivali possono essere causate dallo stress? Non necessariamente. Ma se la lesione non è una bolla gengivale ma un “herpes labiale” causato da un ceppo del virus dell'herpes, lo stress può esacerbarlo e farlo riacutizzare a causa di un indebolimento del sistema immunitario.



Le bolle gengivali possono farti ammalare? Sì. Qualsiasi infezione orale, specialmente bolle gengivali o ascessi dentali, può diffondere germi in altre parti del corpo. È noto che il pus di una bolla gengivale può persino diffondersi al cervello.



Gli ormoni possono causare bolle gengivali? Non è raro vedere qualcosa chiamato “tumori della gravidanza” (che possono assomigliare a bolle gengivali) sulle gengive durante la gravidanza. Il gonfiore localizzato è solitamente temporaneo e dovuto alle fluttuazioni ormonali durante la gestazione e scompare dopo il parto. Tuttavia, se si tratta di un'infezione gengivale non trattata, i tumori della gravidanza potrebbero aumentare il rischio di parto prematuro e preeclampsia.



Gli antibiotici possono causare bolle gengivali? Gli antibiotici sono solitamente usati per trattare le bolle gengivali, riducendo l'infezione e l'infiammazione prima del trattamento canalare (rendendo la terapia endodontica più confortevole). Se sei allergico a qualche tipo specifico di antibiotico, informa il tuo medico o dentista.



Le bolle gengivali diventano più grandi? Le bolle gengivali possono avere forme e dimensioni diverse. Per alcune persone, possono essere grandi quanto la punta di uno spillo. Per altre, l'entità del gonfiore e del pus all'interno della fistola può far sì che la bolla raggiunga la dimensione di una biglia o più.



Le bolle gengivali guariscono da sole? Se la bolla gengivale scoppia improvvisamente o sembra scomparire, non significa necessariamente che l'infezione sia guarita. Potrebbe esserci ancora un ascesso dentale profondo sotto le gengive, all'interno della radice del dente. Invece di avere una bolla gengivale, potresti avere solo una piccola apertura da cui fuoriesce il pus.



I denti del giudizio possono causare ascessi gengivali? È abbastanza comune che i denti del giudizio si infettino intorno a loro, causando cisti ingrandite, gonfiore e dolore. A causa della loro posizione (e della difficoltà di pulizia), i denti del giudizio si infettano più facilmente rispetto agli altri denti, portando allo sviluppo di ascessi gengivali.



Quanto tempo ci vuole per guarire un ascesso gengivale? Il tempo necessario per la guarigione di un ascesso gengivale dipende spesso dalla rapidità e dal tipo di trattamento ricevuto. Di solito ci vogliono circa un paio di settimane perché l’infezione (inclusi eventuali gonfiori o dolori correlati) scompaia con il trattamento adeguato.
