Lentiggini solari è il termine medico per le macchie senili, talvolta chiamate macchie del fegato. Si manifestano più spesso sulla pelle che ha avuto la maggiore esposizione al sole: viso, collo, mani, braccia, spalle e parte superiore della schiena. Le macchie senili sono di solito piatte e di colore marrone, nero o grigio. Sono comuni dopo i 40 anni, ma possono comparire anche in persone più giovani. Alcune possono avere un aspetto simile a condizioni mediche più gravi. 

Le macchie senili sono innocue, ma possono destare preoccupazioni riguardo al cancro. Le lesioni che compaiono spontaneamente, cambiano forma o crescono rapidamente dovrebbero essere controllate per bordi irregolari e pigmenti variabili all'interno della stessa lesione.

Come schiarire le macchie senili

Per motivi cosmetici, le macchie senili possono essere schiarite con agenti sbiancanti per la pelle o rimosse chirurgicamente. La migliore gestione delle macchie senili è prevenirle usando la protezione solare ed evitando il più possibile l'esposizione diretta al sole. Scegliere creme solari naturali che non contengano parabeni o petrolchimici.  

Cause

Le macchie senili sono causate principalmente da anni di esposizione alla luce ultravioletta (UV) del sole. Anche le lampade e i lettini abbronzanti commerciali danneggiano la pelle. La melanina è il pigmento presente nello strato superiore della pelle (epidermide), che conferisce alla pelle il suo colore normale. La luce UV accelera la produzione di melanina, causando l'abbronzatura, che protegge gli strati più profondi della pelle dalla luce UV.

Le macchie senili si formano quando la melanina "si raggruppa" o è, in alcune aree, in concentrazione particolarmente alta. Anche l'invecchiamento può accelerare la produzione di melanina, anche senza esposizione al sole. Può esserci un effetto genetico, con alcune persone più suscettibili di altre a formare macchie senili. Le persone con pelle chiara sono più inclini a sviluppare macchie senili rispetto a quelle con pelle naturalmente più scura. Vi è anche un rischio aumentato in caso di storia di esposizione solare eccezionalmente intensa e frequente. (1,2)

Le lentiggini solari sono: 

  • maculare - un'area piatta e delimitata di aumento della pigmentazione cutanea che non è né rilevata né depressa.
  • di solito marroni, nere o grigie.
  • si trovano sulla pelle che ha avuto la maggiore esposizione al sole nel corso degli anni, solitamente sul dorso delle mani, sulla parte superiore dei piedi, sul viso, sulle spalle e sulla parte superiore della schiena.
  • variabili in dimensione, da 1 millimetro a 3 centimetri di diametro.
  • tendono a raggrupparsi, rendendole più evidenti.
  • a volte simili nell'aspetto a lesioni maligne o non maligne.
  • più probabile che si verifichino in persone bianche o asiatiche, specialmente in individui con tendenza a sviluppare lentiggini.

Il melasma e le efelidi sono due tipi di lesioni maculari iperpigmentate non maligne che possono essere confuse con le macchie senili. Il melasma è un'iperpigmentazione progressiva e non desquamativa della pelle esposta al sole, specialmente sul viso e sugli avambracci.

La formazione del melasma è spesso associata a gravidanza, contraccettivi orali e alcuni anticonvulsivanti. Può comparire spontaneamente senza causa identificabile o associazione a fattori coesistenti noti. Si verifica nelle donne nove volte più frequentemente che negli uomini, e più spesso in tipi di pelle comuni in Asia, Medio Oriente e Sud America. Non ha conseguenze sulla salute, ma può essere fonte di disagio estetico. 

Il viso è il sito più comune del melasma, di solito al centro del viso; meno frequentemente si presenta intorno alla bocca e alla mascella. Le macchie sono solitamente bilaterali. Esistono tre tipi di melasma: epidermico, dermico e misto. Il melasma epidermico è solitamente marrone chiaro e si evidenzia sotto l'esame con lampada di Wood. Il melasma dermico è grigiastro e non si evidenzia con la lampada di Wood. Le lesioni miste sono marrone scuro con risposta variabile alla lampada di Wood. Le efelidi sono il nome medico per le lentiggini, solitamente di 1-2 millimetri, ben definite, sul viso, collo, petto e braccia. (2) 

Le lentiggini solari possono assomigliare a nei (nevi), che possono essere rilevati o piatti, e non sono limitati alle parti del corpo esposte al sole. Le cheratosi seborroiche, che non sono maligne, sono piccole, di colore marrone chiaro, marrone o nero, con un aspetto "incollato". La dimensione normale varia da 1 millimetro a oltre 2,5 centimetri. La lentigo maligna è un tipo di cancro della pelle, un melanoma, che può svilupparsi in aree di esposizione solare prolungata. Inizia come marrone chiaro, marrone o nero, che si scurisce e si ingrandisce. Presenta bordi irregolari ed è multipigmentata all'interno della stessa lesione. Le cheratosi attiniche pigmentate sono premaligne. (2,4) 

Le nuove lesioni cutanee devono sempre essere valutate da un dermatologo certificato. Le seguenti caratteristiche destano preoccupazione:

  • pigmentazione scura
  • crescita rapida
  • bordi irregolari
  • più colori all'interno della stessa lesione
  • sensibilità o sanguinamento
  • prurito, pelle arrossata
  • dolore
  • cattiva guarigione

Qualsiasi lesione atipica o sospetta per melanoma dovrebbe essere sottoposta a biopsia, attraversando tutti gli strati della pelle. Le dermatiti seborroiche e le cheratosi attiniche pigmentate non possono sempre essere distinte solo con la visualizzazione. In caso di dubbio sulla diagnosi, è necessario eseguire una biopsia. (1,2,4) 

Alcuni disturbi sistemici si manifestano inizialmente con multiple lentiggini: sindrome di Peutz-Jeghers, sindrome LEOPARD e sindrome di Lamb.    

Prevenzione

Cura del corpo  
  • Si consiglia di evitare l'esposizione diretta al sole tra le 10:00 e le 16:00, quando i raggi solari sono più intensi. Le attività all'aperto dovrebbero essere programmate prima delle 10:00 o dopo le 16:00.
  • Utilizzare la protezione solare non solo sul viso, ma anche su zone spesso trascurate come il dorso delle mani, può aiutare a prevenire le macchie senili.
  • La protezione solare, che protegge sia dai raggi UVA che UVB, con un fattore di protezione solare di almeno 15, dovrebbe essere applicata 15-30 minuti prima dell’esposizione al sole e poi riapplicata ogni due ore. Si raccomanda l’uso di una protezione solare ad ampio spettro con un fattore di protezione di almeno 30.

  • Copriti. Usa un cappello a tesa larga che offre più protezione rispetto a un berretto da baseball o una visiera. Indossa abiti a trama fitta che coprano braccia e gambe. Sono utili anche i capi progettati per fornire protezione solare. Un fattore di protezione ultravioletta (UPF) di 40-50 nei vestiti è la migliore protezione. Il tessuto può perdere protezione se si allunga, si bagna o viene lavato ripetutamente. (2)

  Nutri quotidianamente le cellule della tua pelle con i nutrienti di cui hanno bisogno per rimanere idratate e luminose. Sebbene le macchie senili non rappresentino una minaccia per la salute, possono essere un problema estetico. Esistono trattamenti per schiarirle o rimuoverle. La pigmentazione si trova nella parte più profonda dell’epidermide, lo strato superiore della pelle. Qualsiasi trattamento volto a schiarire le macchie deve penetrare fino alla parte più profonda dell’epidermide. I trattamenti sono considerati cosmetici, quindi di solito l’assicurazione non li copre. È importante scegliere un medico formato ed esperto nei vari trattamenti considerati.  

  • Farmaci. Creme schiarenti prescritte (idrochinone) usate da sole o in combinazione con retinoidi (tretinoina) e un lieve steroide possono schiarire gradualmente le macchie senili nel corso di mesi. Si consiglia fortemente l’uso di una protezione solare naturale con SPF 30 (senza parabeni) durante il trattamento farmacologico. Può causare arrossamento, bruciore, secchezza o prurito.
  • Terapia laser. La terapia laser distrugge i melanociti, le cellule produttrici di pigmento, senza danneggiare la superficie della pelle. Di solito sono necessarie più sedute. Dopo il trattamento, le macchie senili svaniscono gradualmente, nel corso di settimane o mesi. Ci sono pochi effetti collaterali, ma può verificarsi una leggera discromia della pelle.
  • Crioterapia (congelamento). Si utilizza azoto liquido o un altro agente congelante per distruggere il pigmento in eccesso. Viene solitamente usata su una singola macchia senile o su piccoli gruppi di macchie. Può irritare la pelle e c'è un leggero rischio di cicatrici permanenti o discromie.
  • Dermabrasione. Questo trattamento consiste nel levigare la superficie della pelle usando una spazzola a rotazione rapida; la nuova pelle sostituisce quella vecchia. Possono verificarsi arrossamenti temporanei e formazione di croste.
  • Peeling chimico. Questo metodo di trattamento prevede l'applicazione di acido sulla pelle, che brucia lo strato esterno della pelle. Man mano che la pelle si sfalda, si forma nuova pelle. Di solito sono necessari diversi trattamenti per vedere risultati. È fortemente consigliata la protezione solare dopo il trattamento con peeling chimici. Ci sono irritazioni temporanee e un lieve rischio di discromie.
  • Acido naturale. L'uso quotidiano di un acido efficace e naturale come l'acido lattico (che deriva dal latte) può aiutare a trattare l'iperpigmentazione. Può essere usato quotidianamente in un regime di cura domiciliare a basse percentuali, oppure in percentuali più alte in un peeling chimico mensile. Tuttavia, gli acidi dovrebbero essere usati solo sotto la supervisione di un professionista della cura della pelle autorizzato, come un'estetista o un dermatologo. Gli effetti collaterali includono arrossamento, qualche fastidio o sensazione di bruciore, secchezza e maggiore sensibilità al sole.

Stile di vita e rimedi casalinghi

Creme e lozioni schiarenti sono disponibili da banco nei grandi magazzini e nelle farmacie, così come su internet. Possono migliorare l'aspetto delle macchie dell'età nel corso di mesi. I risultati dipendono in parte da quanto sono scure le macchie e dalla frequenza di applicazione della crema o lozione. Il prodotto usato può contenere idrochinone, deossiarbutina, acido glicolico o acido kojico, che possono causare irritazione cutanea. (4,2)

Le pillole anticoncezionali influenzano le macchie dell'età? Gli anticoncezionali orali non hanno effetto sulle macchie dell'età. Possono causare melasma, un'iperpigmentazione di alcune aree del viso, che può essere scambiata per macchie dell'età. 

Dopo il trattamento, le macchie dell'età ricompariranno? Dopo che le lesioni sono state rimosse o schiarite con i vari trattamenti descritti in questo articolo, le stesse macchie non ricresceranno. Rimangono però i cambiamenti cutanei danneggiati di anni fa che hanno portato alla formazione delle macchie trattate. Possono comparire nuove macchie dell'età. 

Gli antibiotici eliminano le macchie dell'età? Non c'è alcuna correlazione tra antibiotici e macchie dell'età, né causano né curano le macchie. 

Sono un fumatore. Che effetto ha questo sulle macchie dell'età? Il fumo danneggia ogni cellula del corpo in modi negativi. Compromette la capacità del corpo di riparare le cellule danneggiate e contribuisce alla formazione delle macchie dell'età.

Quanto tempo dovrei dare ai prodotti sbiancanti da banco prima di consultare un medico? Può volerci anche qualche mese per vedere miglioramenti usando rimedi casalinghi. Un dermatologo certificato può aiutare con prescrizioni e trattamenti laser.

I danni possono verificarsi a qualsiasi età, e l'esposizione al sole continua ad aumentare il rischio di formazione di nuove macchie dell'età e altri problemi cutanei, incluso il rischio di tumori della pelle. 

Cosa posso usare per prevenire le macchie dell'età mentre uso ancora l'abbronzatura artificiale? Non c'è nulla da fare o usare che possa ridurre i danni alla pelle continuando l'esposizione alle lampade delle cabine abbronzanti.
