Lentiginosas solares es el término médico para las manchas de la edad, a veces llamadas manchas hepáticas. Ocurren con mayor frecuencia en la piel que ha tenido mayor exposición al sol: la cara, el cuello, las manos, los brazos, los hombros y la parte superior de la espalda. Las manchas de la edad suelen ser planas y de color marrón, negro o gris. Son comunes después de los 40 años, pero pueden aparecer en personas más jóvenes. Algunas pueden tener una apariencia similar a una condición médica más grave. 

Las manchas de la edad son inofensivas, pero pueden generar preocupación por cáncer. Las lesiones que aparecen espontáneamente, cambian de forma o crecen rápidamente deben revisarse para detectar bordes irregulares y pigmentos variados dentro de la misma lesión.

Cómo aclarar las manchas de la edad

Por razones cosméticas, las manchas de la edad pueden aclararse con agentes blanqueadores de la piel o pueden eliminarse quirúrgicamente. La mejor forma de manejar las manchas de la edad es prevenirlas usando protector solar y evitando la exposición directa al sol tanto como sea posible. Elija protectores solares naturales que no contengan parabenos ni petroquímicos.  

Causas

Las manchas de la edad son causadas principalmente por años de exposición a la luz ultravioleta (UV) del sol. Las lámparas y camas de bronceado comerciales también dañan la piel. La melanina es el pigmento en la capa superior de la piel (epidermis), que le da a la piel su color normal. La luz UV acelera la producción de melanina, causando un bronceado, que protege las capas más profundas de la piel de la luz UV.

Las manchas de la edad se forman cuando la melanina "se agrupa" o está, en algunas áreas, en una concentración particularmente alta. El simple envejecimiento también puede acelerar la producción de melanina, incluso sin exposición al sol. Puede haber algún efecto genético, con algunas personas más susceptibles que otras a formar manchas de la edad. Las personas de piel clara son más propensas a formar manchas de la edad que las personas con piel naturalmente más oscura. También hay un mayor riesgo con antecedentes de exposición solar excepcionalmente intensa y frecuente. (1,2)

Las lentiginosas solares son: 

  • macular: un área plana y delimitada de aumento de pigmentación en la piel que no está ni elevada ni deprimida.
  • usualmente son marrones, negras o grises.
  • se encuentran en la piel que ha tenido mayor exposición al sol durante años, usualmente en el dorso de las manos, la parte superior de los pies, la cara, los hombros y la parte superior de la espalda.
  • de tamaño variable, desde 1 milímetro hasta 3 centímetros de diámetro.
  • tienden a agruparse, haciéndolas más prominentes.
  • a veces similares en apariencia a lesiones malignas o no malignas.
  • es más probable que ocurran en personas blancas o asiáticas, especialmente en personas con tendencia a tener pecas.

El melasma y las efélides son dos tipos de lesiones maculares hiperpigmentadas no malignas que pueden confundirse con manchas de la edad. El melasma es una hiperpigmentación progresiva y no escamosa de la piel expuesta al sol, especialmente en la cara y los antebrazos.

La formación del melasma a menudo está asociada con el embarazo, anticonceptivos orales y algunos anticonvulsivos. Puede aparecer espontáneamente sin causa identificable o asociación con factores coexistentes conocidos. Ocurre en mujeres nueve veces más frecuentemente que en hombres, y más a menudo en tipos de piel comunes en Asia, Medio Oriente y Sudamérica. No tiene consecuencias para la salud, pero puede ser molesto desde el punto de vista cosmético. 

La cara es el sitio más común del melasma, usualmente en el centro del rostro; menos frecuentemente ocurre alrededor de la boca y la mandíbula. Las manchas suelen ser bilaterales. Hay tres tipos de melasma: epidérmico, dérmico y mixto. El melasma epidérmico suele ser marrón claro y se realza bajo la lámpara de Wood. El melasma dérmico es grisáceo y no se realza con la lámpara de Wood. Las lesiones mixtas son marrón oscuro con respuesta variable a la lámpara de Wood. Las efélides son el nombre médico para las pecas, usualmente de 1-2 milímetros, bien definidas, en la cara, cuello, pecho y brazos. (2) 

Los lentigos solares pueden parecerse a los lunares (nevos), que pueden ser elevados o planos, y no se limitan a las partes del cuerpo expuestas al sol. Las queratosis seborreicas, que no son malignas, son pequeñas, de color marrón claro, marrón o negro, con una apariencia de "pegadas". Hay un amplio rango de tamaño normal, desde 1 milímetro hasta más de 2.5 centímetros. El lentigo maligno es un tipo de cáncer de piel, un melanoma, que puede desarrollarse en áreas de exposición solar prolongada. Comienza como marrón claro a marrón oscuro o negro, que se oscurece y agranda. Tienen bordes irregulares y son multipigmentados dentro de la misma lesión. Las queratosis actínicas pigmentadas son premalignas. (2,4) 

Las nuevas lesiones cutáneas siempre deben ser evaluadas por un dermatólogo certificado. Las siguientes características son preocupantes:

  • pigmentación oscura
  • crecimiento rápido
  • bordes irregulares
  • múltiples colores dentro de la misma lesión
  • sensibilidad o sangrado
  • piel enrojecida y con picazón
  • dolor
  • mala cicatrización

Cualquier lesión que sea atípica o sugestiva de melanoma debe biopsiarse, atravesando todas las capas de la piel. Las dermatosis seborreicas y las queratosis actínicas pigmentadas no siempre pueden distinguirse solo con la visualización. Si hay incertidumbre sobre un diagnóstico, se debe realizar una biopsia. Algunos trastornos sistémicos se manifiestan primero como múltiples lentigos: síndrome de Peutz-Jeghers, síndrome LEOPARD y síndrome de Lamb.    

Prevención

Cuidado del cuerpo  
  • Se recomienda evitar la luz solar directa entre las 10 a.m. y las 4 p.m., cuando los rayos del sol son más intensos. Las actividades al aire libre deben programarse antes de las 10 a.m. o después de las 4 p.m.
  • Utilizar protector solar no solo en el rostro, sino también en lugares comúnmente olvidados como el dorso de las manos, puede ayudar a prevenir las manchas de la edad.
  • El protector solar, que protege tanto de los rayos UVA como UVB, con un factor de protección solar de al menos 15, debe aplicarse de 15 a 30 minutos antes de la exposición al sol y luego reaplicarse cada dos horas. Se recomienda usar protector solar de amplio espectro con un factor de protección de al menos 30.

  • Cúbrete. Usa un sombrero de ala ancha que ofrece más protección que una gorra de béisbol o una visera. Usa ropa de tejido apretado que cubra brazos y piernas. La ropa diseñada para proporcionar protección solar es útil. Un factor de protección ultravioleta (UPF) de 40-50 en la ropa es la mejor protección. La tela puede perder la protección al estirarse, mojarse o con lavados repetidos. (2)

  Alimenta tus células de la piel diariamente con los nutrientes que necesitan para mantenerse hidratadas y radiantes. Aunque las manchas de la edad no representan una amenaza para la salud, pueden ser una preocupación estética. Existen tratamientos disponibles para aclararlas o eliminarlas. La pigmentación está en la parte más profunda de la epidermis, la capa superior de la piel. Cualquier tratamiento destinado a aclarar las manchas debe penetrar hasta la parte más profunda de la epidermis. Los tratamientos se consideran cosméticos, por lo que generalmente el seguro no los cubre. Se debe tener cuidado al seleccionar un médico capacitado y con experiencia en los diversos tratamientos que se están considerando.  

  • Medicamentos. Las cremas blanqueadoras con receta (hidroquinona) usadas solas o con retinoides (tretinoína) y un esteroide suave pueden aclarar gradualmente las manchas de la edad durante varios meses. Se recomienda encarecidamente usar un protector solar natural con un SPF de 30 (sin parabenos) cuando se utiliza tratamiento con medicamentos. Puede causar enrojecimiento, ardor, sequedad o picazón.
  • Terapia láser. La terapia láser destruye los melanocitos, las células productoras de pigmento, sin dañar la superficie de la piel. Por lo general, se requieren varias sesiones. Después del tratamiento, las manchas de la edad se desvanecen gradualmente, durante semanas o meses. Hay pocos efectos secundarios, pero puede haber una ligera decoloración de la piel.
  • Crioterapia (congelación). Se utiliza nitrógeno líquido u otro agente congelante para destruir el pigmento excesivo. Generalmente se usa en una sola mancha de la edad o en pequeños grupos de manchas. Puede irritar la piel y existe un ligero riesgo de cicatrices permanentes o decoloración.
  • Dermabrasión. Este tratamiento consiste en alisar la superficie de la piel usando un cepillo que gira rápidamente; la piel nueva reemplaza a la vieja. Puede haber enrojecimiento temporal y formación de costras.
  • Peeling químico. Este método de tratamiento consiste en aplicar ácido sobre la piel, quemando la capa externa. A medida que la piel se desprende, se forma piel nueva. Por lo general, se necesitan varios tratamientos para ver resultados. Se recomienda encarecidamente la protección solar después del tratamiento con peelings químicos. Hay irritación temporal y un ligero riesgo de decoloración.
  • Ácido de origen natural. El uso diario de un ácido efectivo y de origen natural como el ácido láctico (que proviene de la leche) puede ayudar a tratar la hiperpigmentación. Puede usarse diariamente en un régimen de cuidado en casa en porcentajes bajos, o en porcentajes más altos en un peeling químico mensual. Sin embargo, los ácidos solo deben usarse bajo el cuidado de un profesional licenciado en cuidado de la piel, como un esteticista o dermatólogo. Los efectos secundarios incluyen enrojecimiento, algo de incomodidad/sensación de ardor, sequedad y mayor sensibilidad al sol.

Estilo de vida y remedios caseros

Las cremas y lociones aclarantes están disponibles sin receta en grandes almacenes y farmacias, así como en internet. Pueden mejorar la apariencia de las manchas de la edad en el transcurso de meses. Los resultados dependen en parte de qué tan pigmentadas estén las manchas y con qué frecuencia se aplique la crema o loción. El producto usado puede contener hidroquinona, deoxiarbutina, ácido glicólico o ácido kójico, que pueden causar irritación en la piel. (4,2)

¿Las píldoras anticonceptivas afectan las manchas de la edad? Los anticonceptivos orales no afectan las manchas de la edad. Pueden causar melasma, una hiperpigmentación en áreas del rostro, que puede confundirse con manchas de la edad. 

Después del tratamiento, ¿volverán a aparecer las manchas de la edad? Después de que las lesiones son eliminadas o desvanecidas por los diversos tratamientos mencionados en este artículo, las mismas manchas no volverán a crecer. Sin embargo, permanecen los cambios en la piel dañada de años atrás que llevaron a la formación de las manchas tratadas. Pueden aparecer nuevas manchas de la edad. 

¿Los antibióticos eliminan las manchas de la edad? No existe ninguna relación entre los antibióticos y las manchas de la edad, ni causan ni curan las manchas. 

Soy fumador. ¿Qué efecto tiene eso en las manchas de la edad? Fumar afecta negativamente a cada célula del cuerpo. Daña la capacidad del cuerpo para repararse a sí mismo y contribuye a la aparición de manchas de la edad.

¿Cuánto tiempo debo darle a los agentes blanqueadores de venta libre para que funcionen antes de ver a un médico? Puede tomar meses ver alguna mejoría al usar remedios caseros. Un dermatólogo certificado puede ayudar con tratamientos con receta y láser.
